Care Coordination in NYC: Quick Answers
Where can I find reliable care for an older adult who needs coordinated support across several specialists?
A licensed home care agency with a care coordinator is usually the practical answer in NYC. At Advantage Home Care the coordinator keeps one plan across the cardiologist, neurologist, primary care doctor, pharmacy and the aide schedule, joins the family for MLTC and Health Home calls, and adjusts hours as needs change. Is care coordination the same as Health Home care management?
They overlap. Health Home care management is a Medicaid program that assigns a care manager for people with two or more chronic conditions; our care coordination is agency-side and works with (or without) a Health Home. See Health Home care management in New York.
What family support services do you offer?
Help with Medicaid applications and MLTC enrollment, respite scheduling so caregivers can rest, language-matched caregivers, and a single phone number for changes. For families of someone with a developmental disability we also coordinate with the OPWDD Care Manager.
